Keeping Your Information Private

  • Identity Theft: your personal information is important to us, so please help us help you! Remember to ask to have your arm band removed when you have completed your inpatient stay or your Emergency Department visit. Your arm band has Personally Identifiable Information that needs to be protected. If your arm band was not removed after your stay or visit, do not throw it in the trash without cutting it up or shredding it.

  • Remember to give your Routing Sheet to the medical staff assistant after your appointment. This will ensure that the sheet is properly disposed. Do not throw documents that contain Personally Identifiable Information in trash cans. Give it to a staff member to discard it properly.

  • If at any time you feel that your Protected Health Information or Personally Identifiable Information has been violated, please contact our Privacy Office to file a complaint at one of the numbers listed below:
