Patient Driven Scheduling

What is Patient Driven Scheduling? Patient driven scheduling is a system for scheduling your future appointments that are beyond 90-days.

How does Patient Driven Scheduling work?
When you come in for your visit, you and your health care provider will decide when you need to be seen again. The clerk will then give you a letter showing the time frame for your next visit. Additionally, you will receive a reminder notice 30 days before your next visit that will ask you to call the number in the letter to schedule an appointment with your health care provider. If you do not call within 7 to 10 days, a second reminder notice will be mailed to you. If you fail to follow-up, you will be required to call and schedule your appointment when you are ready to be seen.

Will I “fall through the cracks” with this method of scheduling?
No. The reminder system will send you a letter when it is time for you to see your health care provider. A second reminder notice will automatically be sent to you if you do not call to schedule an appointment within two weeks.

What if I need to cancel my appointment?
Please call one of our scheduling clerks if you are not able to keep your scheduled appointment. Please note that if you cancel a specialty clinic appointment more than one time or you fail to keep a scheduled specialty appointment and do not reschedule your appointment at the time of your cancellation, you will need to contact your health care provider for another consult or referral for that specialty clinic.

What about my appointments for lab work or other tests that should be done before I see my health care provider?
The clinic clerk will schedule a date for your lab work or tests when you call to schedule an appointment with your health care provider.
