Perry Point VA Medical Center - Inpatient Services Eligibility Office The Eligibility Office provides one-stop service for Veterans who need to update their insurance, home address, financial and means-test information. If you have a question about your VA health care eligibility or inpatient co-payment billing procedures, the staff in the Eligibility Office will help answer your questions. To contact the Eligibility Office, please call (410) 642-2411, extension 5018, Monday through Friday from 7:30 a.m. to 4 p.m. Pharmacy Services Inpatient Pharmacy – Pharmacists are part of your health care team and are available to provide medication counseling to you, your provider and hospital staff. Pharmacists will provide medication information such as dosage and possible side effects, or make suggestions for different drugs to best treat your condition. They are available to review your medications with you before you are discharged from the hospital. Inform a member of your health care team if you need to speak with a pharmacist. Outpatient Pharmacy – The Outpatient Pharmacy is open Monday through Friday from 8 a.m. to 6 p.m. for new prescriptions only. The Outpatient Pharmacy is located in the main lobby of the medical center on the 1st floor. To request a prescription refill or to check on your prescription status 24 hours a day, 7 days a week, please call the Prescription Refill Line at 1-800-463-6295, extension 7395. Please have your Social Security number and your prescription numbers ready when calling. Pastoral Care and Services The mission of Pastoral Care Service is to provide religious care to all Veteran patients and their family members. Through religious guidance and counseling, chaplains assist patient’s to utilize their beliefs and attitudes in the management of their illness. Chaplains take a holistic approach in the patient care process, which includes support for the seriously ill, pre-surgery patients, the dying patient and family, as well as bereavement counseling. Chaplains visit inpatient units regularly and are available for private consultation. Patients can ask for written materials from Pastoral Care Service to assist them during their stay at the Perry Point VA Medical Center. To arrange a visit by a chaplain or to inquire about available programs, please call Pastoral Care Service at (410) 642-2411, extension 5601. Chaplains representing Protestant, Catholic and Jewish faiths are on call 24 hours a day. Each patient is visited by a chaplain within 48 hours of admission. Seriously ill patients are visited daily. The main chapel is located in building 314C in room 100 and is open from 8 a.m. to 4 p.m. Sunday through Friday. The following is a listing of chaplain services: Protestant Sunday 9 a.m. Catholic Sunday 10:30 a.m. Jewish As Scheduled Voluntary Service Voluntary Service is a program supported by individuals and community groups who voluntarily donate their time and resources to benefit Veteran inpatients and outpatients at the Perry Point VA Medical Center. Volunteers visit most inpatient units on a daily basis and can assist you with letter writing, escort service throughout the facility and other personal services. The goal of the Voluntary Service Program is to provide volunteers and donated items to assist in making your inpatient stay as enjoyable and comfortable as possible. Voluntary Service is located on the second floor of building 15H. For more information about the Voluntary Service Program or to request a volunteer to visit you during your inpatient stay, please call (410) 642-1038. Guest Services Guest Services is available Monday through Friday to deliver comfort kits, stamps, puzzles, models, stationery, pencils, magazines and playing cards directly to your room, courtesy of the Voluntary Service Program, by calling (410) 642-2411, extension 5413 from your beside phone. Recreation Service Through the support of numerous dedicated volunteers and community groups, Recreation Service sponsors such activities as bingos, ward parties, musical entertainment, arts and crafts programs and holiday events. In addition to the numerous activities that they sponsor each week for your benefit, Recreation Service offers games, models and puzzles to each hospitalized Veteran through the Craft Room located in buildings 314 and 24H. The building 314 Craft Room is open Tuesdays from 1 p.m. to 3:30 p.m. and Thursdays from 3 to 5 p.m. and is located in room A112A. The building 24H Craft Room is open Mondays and Wednesdays from 6 to 8 p.m. in room 13. Recreation Service also has a variety of facilities available for your enjoyment during your inpatient stay at the medical center, including a bowling alley, music room, sports hut, game room and gymnasium. Disabled American Veterans (DAV) Transportation Program Eligible Veterans who are in need of transportation to and from clinic appointments at the Baltimore VA Medical Center can contact the DAV Transportation Program Monday through Friday from 8 a.m. to 3 p.m. at (410) 642-2411, extension 5340. The DAV Transportation Program is located in building 6H in room 6. To be eligible for transportation services with the DAV Transportation Program, Veterans must meet the following criteria:
Requests for transportation to the Perry Point VA Medical Center should be made at least 72 hours in advance of scheduled clinic appointments. The DAV Service Coordinator will arrange transportation via government vehicle based on availability of vehicles, authorized volunteer drivers and seating space. Canteen The Canteen Cafeteria, Retail Store and Vending Room are located in building 17H. The Canteen Cafeteria and Retail Store are closed on Saturday, Sunday and holidays. Inpatients must obtain permission from a member of their health care team and sign out in the logbook at the Nursing Station before leaving their inpatient unit. The Canteen Retail Store offers clothing, personal care items, snacks and a variety of specialty items. The Canteen Cafeteria offers home-style meals, sandwiches, soups, salads and beverages. The following is a listing of the hours of operation for the Canteen Cafeteria, Retail Store and Vending Room: HOURS OF OPERATION FOR PATIENTS Cafeteria Monday-Friday 7 a.m. to 2:30 p.m. Retail Store Monday-Friday 7:30 a.m. to 3:30 p.m. Vending Areas Daily 24 Hours Barber Service The barbershop is open on Tuesday and Thursday from 7:15 a.m. to 2:30 p.m. The barbershop is located in Building 17H. Canteen Books Canteen Books are available for patients to use during their hospital stay. They can be purchased in the Canteen Retail Store. Canteen Books can be used in the Canteen Cafeteria and Retail Store. Patients and family members are encouraged to purchase these books instead of keeping money at the bedside. Shuttle Services There is a free shuttle service to and from the Baltimore VA Medical Center that is available for patients Monday through Friday. The shuttle departs from in front of building 361 at 6:30 a.m., 8 a.m., 9:30 a.m., 10:45 a.m., 12 p.m., 2 p.m., 3 p.m. and 4:45 p.m. Patients are required to sign-up for the shuttle to the Baltimore VA Medical Center by calling (410) 642-2411, extension 6050. Patient Education Closed Circuit Television – Closed Circuit Television service is available in all clinics and in most patient rooms throughout the Perry Point VA Medical Center. The system features a variety of special TV programs, including a health information channel that is available to patients 24 hours a day, 7 days a week on channel 8. Absentee Voting Veterans who expect to be hospitalized for an extended time and are registered voters can get assistance from their social worker to apply for an absentee ballot. The procedure must be started a few weeks prior to Election Day. Please inform a member of your treatment team if you need to get an absentee ballot from your social worker. Release of Information Release of all medical information (labs, x-rays, hospital records, disability and/or insurance forms) must be referred through the Release of Information/Medical Records Section, which is located in building 361 in room GA-114 Release of Information is open Monday through Friday from 8 a.m. to 4:30 p.m. You can contact the office directly at (410) 642-2411, extension 6028. Interpreter Services Interpreter services for deaf and speech-impaired veterans are available. Please contact a member of your health care team to arrange for interpreter assistance.
